Financial Information
Our entire staff is dedicated to deliver the finest and most comprehensive periodontal and implant services available today. In order to assist you with the investment in your dental health, we have outlined our payment policy.
As with any business, Full Payment is expected at the time the services are rendered. Full payment may come in the form of direct payment from the patient and/or insurance reimbursement.
We will be happy to assist in the processing of your insurance claim. It is ultimately the patients responsible to pay the portion of the bill not paid by their insurance company (unless otherwise restricted by law or agreement we might have with the insurer).
For your convenience, payments may be made with cash, personal check, bank draft, Discover, Visa, Master Card or Care Credit.
Pre-Payment Discount
A 5% fee reduction is offered to any patient making payment in FULL with cash or check before treatment is rendered.
Insurance patients paying the full amount of their surgery/treatment by CASH or CHECK will receive a 5% discount on the services provided. Even if you have insurance you may pay the full balance for your surgery to receive the discount and then file insurance on your own. We will give you a completed claim form.
Insurance Patients
Patients are responsible for paying the amount in excess of the insurance estimate on the day of surgery/treatment. We will submit a claim to your insurance company for the treatment performed.
Monthly Payments
Our office is proud to offer Care Credit for patients desiring a more flexible payment option. The approval process is simple and can be accomplished in our office or in the privacy of your home at www.carecredit.com. Once approved, Care Credit will remit payment to our office and you will repay them monthly. Care Credit offers NO INTEREST options ranging from 6-24 months to repay.
